JANICE HINES MEMORIAL FUND

1. Purpose:
   To support the transfer of knowledge of best practices in paediatric occupational therapy - $1,000 (1 available annually)
   
   
2. Eligibility:
   The Applicant must:
   • be a CAOT member in good standing
   • demonstrate the ability to transfer knowledge of best practices
   • demonstrate the relevance to paediatric occupational therapy practice
   
   
3. Guidelines:
   The application may be used for, but is not limited to, the costs incurred for the following:
   • visit/travel to other sites
   • presenting at an appropriate conference/research meeting
   • prototype development of equipment based on client centered needs
   • development of continuing professional education programs for use within the occupational therapy community
   
   
4. Documentation: (3 copies of each document)
   4.1 A photocopy of CAOT membership card
   
   4.2 Completed and signed application form
   
   4.3 Application letter, not in excess of 1 (8.5 x 11) typed page (single spaced, using 1 inch margins with 11-point print size). The letter must include:
   • projected time line
   • relevance to paediatric practice of the profession
   • contribution to the transfer of knowledge and practice of paediatric occupational therapy
   
   4.4 A copy of your curriculum vitae
   
   
5. Deadline:
   SEPTEMBER 30, ANNUALLY. Applications not received in the COTF office by the specified date & time will be returned to the applicant.
