Reporting Forms
To further its awareness and fundraising activities, COTF will publicize information about its award recipients and their projects. All award recipients must complete the the COTF Annual Award Recipient's Report and the COTF Award Recipient's Project Completion Report (at the termination of the funded project), as conditions of accepting an award from COTF to further assist COTF in publicity initiatives. If a project has not been completed within twelve months of the receipt of an award, the award recipient must notify COTF in writing, stating the reasons for the delay and requesting an extension.
